New Patients
Initial Appointment
Your initial appointment usually will consist of an examination, full mouth x-rays, dental cleaning and a review of your treatment options. Prior to scheduling any additional treatment, our front office will provide you with a complete treatment plan, including your estimated out-of-pocket cost for each visit.
Please assist us by providing the following information at the time of your consultation:
- The name and phone number of your previous dentist, or a copy of your most current x-rays, taken within the previous five years. We are happy to make a records transfer request for our patients, if preferred.
- Please complete the Health History and Registration form that we provide by mail or email, and sign the HIPAA Privacy Practice and Dental Material Fact Sheet attached.
- Please alert the office if you have a medical condition or allergy that may be of concern prior to treatment.
- A copy of your dental insurance card, or the name and phone number of your insurance provider, if applicable.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or legal guardian.
